How to File a Business Corporation Annual Report Form (RI Form 630)

In order for this form to be considered complete, all sections, including the signature and date fields, must be filled in. If the response is none to any of the fields, please write “none” instead of leaving it blank. If any section on the form is not completed, the form will be returned to you. Black ink must be used when filling out the form and it must either be typed or legibly printed. An authorized representative is in charge of executing this file on behalf of the corporation. It can also be filed by a receiver or trustee.
As an annual report, this form must be submitted every year between January 1st and March 1st. The report may not be submitted more than sixty (60) days before the end of the filing period. If this occurs, the corporation may face revocation of the Certificate of Incorporation or the Certificate of Authority.
Even if the corporation is not actively engaged in business at the time the report is due, it still must file.
